What is root cause analysis? 

Root cause analysis is the process of discovering the root causes of problems to identify appropriate solutions. Root cause analysis it the thought of that it’s more effective to prevent the issue rather than tackling it when the issue arrives. 

 

 

5 Whys 

The 5 whys are used to explore the relationship between the cause and effect of a problem. The main goal of the 5 whys is to find the root cause of a problem by repeating the question “why?”. Once you’ve got to the last of the why’s, that’s when you’ve found the root cause.  


The 5 why’s could be: 

  • Why is it happening? 

 

  • Why is that? 

 

  •  Why is that? 

 

  • Why is that? 

 

  • Why is that?

Fishbone diagram 

A fishbone diagram is a visual way to look at cause and effect. Compared to other tools to find for finding causes of a problem, like the 5 whys, it is a more structured approach. The problem is often found at the mouth of the fish diagram. Scenarios where a fish bone diagram may come in handy may include, product development and troubleshooting processes. The centre of the fish, (the bone structure), is the place where the brainstormed ideas are held. These could consist of anything from the methods they could use, the budget they have, the machinery they have access to.  

 

 

 

 

 

 


Failure mode and effects analysis (FMEA)

Failure mode and effects analysis (FMEA) is a structured way to identify and address potential problems. This is one of the most basic forms of root cause analysis. It also estimates the potential severity or impact of a root cause analysis (affect). It is a basically a step-by-step approach for collecting knowledge about possible points of failure in a design, manufacturing process, product or service. Many people use FMEA daily. This is when people plan to do, design or make something and they look for potential failures in the process. 

 


Event tree analysis (ETA)

Event tree analysis (ETA) is a procedure that shows all possible outcomes resulting from an accidental event, considering whether installed whether installed safety barriers are functioning or not. An event tree considers the impact of the failure of a particular component or item in the system and works out the effect such a failure will have on the overall system risk or reliability. 

 


Pareto chart 

A Pareto chart is a type of chart that contains both bar and line graphs, where individual values represented in descending order by bars, and the cumulative total is represented by the line. 

 

Scatter Diagram 

A scatter diagram or scatter plot is a type of chart used to show a trend in various sectors of the project. A scatter diagram is plotted against 2 mathematical values on to a 2D graph. A scatter diagram can also show any anomalies and anomalous trends to find the root cause of problems and improve the necessary areas. 

 

 

 

 

When is root cause analysis used? 

Root cause analysis is used to break down a problem into multiple different smaller parts to find the finer causes which could be the root cause. Businesses will use this to find the root cause of a problem. It will help their business become more efficient and effective when dealing with problems and overall productivity.
